Visus Therapeutics is operated by Visus Therapeutics, Inc., a Delaware corporation (“Visus”) with its principal place of business in the state of Washington. What information does Visus collect about me as I use the Visus Online Services?
Visus may collect personally identifiable information from you such as your name, telephone number, postal address, e-mail address, location, and credit card information so that we or selected companies can provide you with the products and services that you request (“Personal User Information”). For example, we may ask you for your name, e-mail address, and certain other information in connection with certain features related to contacting the Visus team or providing feedback. Visus may also collect non-personally identifiable information when you visit the Visus Website or use other Visus Online Services, such as your browser configuration, the type of operating system you are using and information about your internet service provider (“General User Information” and together with any and all Personal User Information, “User Information”).
How does Visus use the information supplied by me?

Is the Visus Website Health Insurance Portability and Accountability Act compliant?
The Visus Website and other Visus Online Services are not required to comply with the Health Insurance Portability and Accountability Act (“HIPAA”) as Visus does not collect or maintain records of any health information, as defined under HIPAA. How can I change my privacy options?
If at any point after giving Visus your Personal User Information, you do not wish to continue to receive additional information from or contact by Visus, Visus’ affiliates, or selected outside companies, you may opt out of receiving future emails by contacting Visus directly at webmaster@visustx.com.
What are “cookies” and how does Visus use them?
“Cookies” are pieces of information that are placed by the Visus Online Services in a storage place on your computer or mobile device. Cookies do not contain information that is, in itself, personally identifying. Cookies and other tracking information are placed on your computer or mobile device in a limited way to help deliver advertisements, to calculate unique site visitors, and to track usage throughout our site. We use the services of a third party marketing company, to help us measure the effectiveness of our advertising and how visitors use the Visus Online Services. To do this, we use Web beacons and cookies provided by this third party marketing company at the Visus Online Services.
Visus uses a third party web developer, DiD Agency Group, LLC, to design and host the Visus Website and other relevant content related to the Visus Online Services. Our web developers may store and have access to all User Information. DiD Agency does not use any User Information provided by customers for any purpose other than the development and operation of the Visus Website.
Children Under Age 13
No one under the age of 13 should provide any Personal User Information to Visus. Visus does not knowingly collect Personal User Information from children under 13. In the event that we learn that we have collected such Personal User Information from a child under the age of 13 without verification of parental consent, we will delete this information from our database as quickly as possible. If you believe the Visus Online Services has collected any Personal User Information about anyone under 13, please contact us at webmaster@visustx.com.
California Residents
Users of the Visus Online Services who are California residents and who have provided Personal User Information to us may request certain information regarding our disclosure of Personal User Information to third parties for their direct marketing purposes.
